1. Records Management
Ensure proper creation, classification, storage, and retention of patient medical records.
Implement policies for confidentiality, security, and proper usage of records.
Manage both electronic and paper-based health record systems (EMR/HIMS).
2. Supervision and Staff Management
Supervise and train record room staff, coders, and clerks.
Assign daily tasks and monitor performance.
Ensure timely completion of discharge summaries and clinical documentation.
3. Compliance and Legal Responsibilities
Ensure all records comply with HIPAA (or local laws), accreditation standards (e.g., NABH, JCI), and institutional protocols.
Facilitate medico-legal documentation and reporting.
4. Data Coding and Indexing
Oversee ICD (International Classification of Diseases) coding for diagnoses and procedures.
Ensure accuracy for statistical reporting, insurance claims, and research.
5. Quality Control and Audits
Conduct regular audits for accuracy, completeness, and compliance.
Identify gaps and provide corrective training.
6. Academic and Research Support
Provide data and records for clinical studies, audits, case studies, and journal publications.
Maintain confidentiality while supporting research access.
7. Interdepartmental Coordination
Liaise with clinicians, nurses, IT staff, and administrative departments.
Ensure timely collection and submission of discharge summaries, test reports, and case sheets.
8. Digital Transformation & System Maintenance
Work with IT teams to maintain Electronic Medical Record (EMR) systems.
Ensure backup, system updates, and data integrity.
