He/she is office-based and responsible for providing administrative support to the clinical team as they provide cost-effective, high-quality health care for patients.The Care Coordinator will be responsible for ensuring patients and Providers have exceptional in-home visit experiences. He/she will provide office administrative support for the HBACP program and its providers.
