Role: CARE COORDINATORThis role will be for a Middle Eastern Client who will be running care management programs.Mode: Remote opportunityDays of work: Sunday - Thursday (According to Middle East working days)Role Summary: The Primary Care, Care Coordinator is a critical member of the Primary Care team. He/she is office-based and responsible for providing administrative support to the clinical team as they provide cost-effective, high-quality health care for patients.The Care Coordinator will be responsible for ensuring patients and Providers have exceptional in-home visit experiences. He/she will provide office administrative support for the HBACP program and its providers. This position is responsible to the clinical team and reports to the Care Coordination Supervisor.ROLE RESPONSIBILITIES

  • Answers incoming phone calls for general inquiries and redirect as appropriate
  • Route incoming faxes and assign as appropriate
  • Assist with the implementation or finalization of patient orders that do not require an RN to complete.
  • Document results and patient information in Electronic Medical Records system
  • Coordinate patient appointments with family members, caregivers and healthcare proxies.
  • Facilitate medical records management for outside documents that are critical for patient management, i.e. laboratory results, discharge summaries, medical documentation from specialists and primary care provider.
  • Support a variety of data entry functions.
  • Support scheduling appointments and reminder calls or texts for chronic care management appointments.
  • Assist with referrals and referral management.
  • Assist with follow-up to assure needed DME is ordered and delivered.
  • Provide oversight for all Care Coordination functions including but not limited to orders, referrals, disenrollment, scheduling, error processing, medical records and consent uploading, mail retrieval and sorting, email processing, etc.
  • Help train new hires and refresher training as requested by management.
  • Ability to take on other Care Coordination team support functions based on management requests.
  • Complete other duties that support the primary care team as assigned.
QUALIFICATIONS
  • Minimum of a High School diploma.
  • Exceptional verbal and written Communication skills.
  • Fluency in Arabic language will be an added advantage.
  • Understanding of medical terminology.
  • Completion of a Medical Terminology course preferred.
  • Medical office experience preferred.
  • Strong work history.
  • Working knowledge of computers and ability to manage data within an electronic system.
