Conduct initial evaluations of potential transplant recipients, including reviewing medical records, conducting interviews, and assessing the patient's overall health and suitability for transplantation.
Coordinate all aspects of the organ or tissue procurement and transplantation process, including working with organ procurement organizations (OPOs), transplant centers, and other healthcare facilities to ensure timely and successful transplantation.
Collaborate with the multidisciplinary team to develop and implement individualized transplant plans for patients, considering their specific medical, psychosocial, and logistical needs.
Educate and support patients and their families throughout the transplant process, providing information about the procedure, potential risks and benefits, and post-transplant care requirements.
Facilitate communication and collaboration between the transplant team, referring physicians, and other healthcare providers involved in the patient's care.
Ensure compliance with regulatory and accreditation standards related to transplantation, including maintaining accurate and complete documentation of all transplant-related activities.
Monitor and track patient outcomes, including graft and patient survival rates, and identify areas for improvement in transplantation processes and patient care.
Stay up to date with advancements in transplantation medicine and research, attending conferences and training sessions as required.
Provide emotional support to patients and their families, addressing any concerns or anxieties they may have throughout the transplant process.
Participate in quality improvement initiatives and contribute to the development and implementation of policies and procedures related to transplantation.
