The role involves patient onboarding in Carely application portal, scheduling condition-based templates, daily follow-up calls to moniter patients' health condition, alert handling, clinical coordination & escalation, and subscription renewal counselling.
Key Responsibilities
Onboard patients using Discharge summaries/OPD prescriptions
Handle patient alerts and follow-ups
Make scheduled daily calls to patients/attendants
Coordinate with hospital doctors for clinical or emergency escalation
Counsel patients for subscription renewals
Maintain call logs and reports using MS Excel and other CRM based applications
Eligibility
Female candidates only
Qualification:
ANM / GNM Nursing
(Mandatory)
Experience:
1-2 years in Hospitals / Tele-Nursing / Patient counselling preferred
Skills:
MS Excel (Mandatory); calling CRM based apps knowledge preferred
Languages:
Odia, English & Hindi (Bengali is an advantage)
