Responsibilities

Patient Care

: Provide direct nursing care to patients in their homes, including administering medications, monitoring vital signs, and managing treatments as prescribed by physicians.

Health Monitoring

: Assess and document patients' health conditions, including changes in symptoms and responses to treatments, and communicate findings to the healthcare team.

Personal Care Assistance

: Assist patients with daily living activities such as bathing, dressing, and meal preparation, ensuring their comfort and dignity.

Education and Support

: Educate patients and their families about health conditions, treatment plans, and preventive health measures, providing emotional support as needed.

Collaboration

: Work closely with other healthcare professionals, including physicians and therapists, to develop and implement comprehensive care plans tailored to individual patient needs.

Documentation

: Maintain accurate and up-to-date records of patient care, including progress notes and treatment plans, ensuring compliance with healthcare regulations.

Qualifications

Education

: A bachelor's degree in nursing (BSN) or an Associate degree in Nursing (ADN) is typically required. A valid nursing license is essential.

Experience

: Previous experience in home health care or a similar setting is preferred.

Skills

: Strong clinical skills, excellent communication abilities, empathy, and the capacity to work independently in a home setting.
