Job Description

Clinical Care



1. Provide comprehensive outpatient and inpatient nursing care to patients attending the Palliative Care Dept at CVRGH.

Assess, plan, implement, and evaluate individualized nursing care for patients receiving palliative care at end-of-life care. Manage pain and symptoms effectively using evidence-based practices. Administer medications safely and monitor for side effects and effectiveness. Provide emotional and psychological support to patients and their families. Coordinate with physicians, social workers, and other members of the multidisciplinary team to ensure holistic patient care. Participates in home health and community OPDs.
2. Conduct home visits for patients with complex palliative care needs, assessing their environment and providing tailored care interventions.

3. Evaluate patient progress and assist in delivery of telemedicine services, ensuring the maintenance of accurate and up-to-date patient records.

4. Partner with departments within the hospital for the management of patient

5. Maintain current nursing registration and comply with all professional and ethical standards.

Administrative Duties



1. Facilitate adequate provisioning of essential drugs and consumables by monitoring stocks and making projections for future needs.

2. Ensure safekeeping of drugs including narcotics, consumables, instruments and other property of OBLF at the secondary unit

3. Adhere to infection control, safety, and hospital policies and procedures.

Education & advocacy



1. Educate patients and families on palliative care principles and symptom management.

2. Support other nursing staff and students as required.

3. Advocate for the dignity, comfort, and preferences of patients throughout their care journey.

4. Participate in staff education and training programs to enhance palliative care knowledge and skills.

Communication & Coordination



1. Participate in training, heath education and discharge planning.

2. Maintain clear, accurate, and timely documentation in patient records.

3. Communicate sensitively with patients and families regarding prognosis, care plans, and treatment options.

Community Engagement



1. Support outreach activities in the community to help identify and serve patients with Palliative Care needs.

2. Create relationships with elected representatives, public officials, and other stakeholders to create awareness and generate support for PC.

Collaboration with the Public Health System



This job involves working closely with the public health system without being directly integrated to it. The nurse should demonstrate resourcefulness and adaptability in overcoming bureaucratic challenges, and fostering a culture of collaboration and teamwork to achieve optimal patient outcomes within the constraints of the public health system. Through all this, the nurse is expected to uphold ethical and professional standards and advocate for the needs of palliative care patients within and outside the healthcare system.

Pay: ₹20,000.00 - ₹40,000.00 per month
