Job Description

The Social Worker in a Rehabilitation Center plays a vital role in supporting patients recovering from physical, mental, or substance-related challenges. They work closely with medical professionals, therapists, and families to ensure holistic rehabilitation and reintegration of clients into society.

Key Responsibilities:

Assess the social, emotional, and psychological needs of clients. Develop individualized care and rehabilitation plans in collaboration with the treatment team. Provide counseling and emotional support to patients and their families. Facilitate group therapy, awareness sessions, and motivational programs. Assist clients in accessing government schemes, financial aid, or vocational training. Maintain case records, progress reports, and documentation as per organizational standards. Coordinate with community organizations and referral agencies for follow-up support. Conduct home visits when required to assess living conditions and family support systems. Promote social reintegration and community participation for recovered clients.

Required Qualifications and Skills:

Bachelor's or Master's degree in Social Work (BSW/MSW). Good communication, empathy, and interpersonal skills. Knowledge of counseling techniques and rehabilitation processes. Ability to work with multidisciplinary teams (medical, psychological, and vocational). Basic computer and documentation skills.
