The Social Worker will provide emotional, social, and practical support to residents in the rehabilitation home. The role focuses on helping individuals recover, rebuild life skills, and reintegrate into society. The social worker will work closely with medical, psychological, and therapy teams to ensure holistic care and progress for each resident.
Key Responsibilities:
Conduct social and psychological assessments of residents upon admission.
Develop individualized care and rehabilitation plans based on client needs.
Provide one-on-one counseling and group sessions to support recovery.
Coordinate with families to encourage positive relationships and aftercare planning.
Connect clients with community resources such as housing, education, or employment programs.
Maintain accurate and confidential case notes and progress reports.
Participate in multidisciplinary team meetings and contribute to treatment planning.
Support residents in adjusting to daily routines and achieving personal goals.
Assist in discharge planning and follow-up support for reintegration.
Qualifications and Experience:
Master's degree in Social Work (MSW).
1-2 years of experience
in social work, preferably in rehabilitation, mental health, or substance abuse settings.
Strong communication, empathy, and case management skills.
Knowledge of counseling techniques and community support systems.
Ability to work effectively in a team and handle sensitive situations with professionalism.
Work Environment:
Based in a residential rehabilitation facility.
May require evening or weekend shifts depending on client needs.
