To facilitate comprehensive patient care coordination during hospitalization by serving as a liaison between the patient, family members, clinical team, and hospital services. This includes guiding families through treatment plans, providing psychoeducation, obtaining financial consent, monitoring patient progress, and promoting mental health awareness.
Key Responsibilities:
1. Coordinate all aspects of inpatient care during hospital admission.
2. Explain financial consent, hospitalization procedures, and billing queries to patients and caregivers.
3. Provide psychoeducation to family members about psychiatric illnesses, treatment modalities, and prognosis.
4. Act as a communication bridge between families and psychiatrists, ensuring timely updates and clarifications.
5. Observe and evaluate patients' emotional and behavioral symptoms, report progress to the clinical team.
6. Participate in multidisciplinary team meetings and contribute to treatment planning.
7. Maintain accurate and up-to-date documentation of patient progress, meetings, and consent.
8. Support discharge planning and continuity of care in coordination with outpatient services.
9. Coordinate and conduct training sessions and awareness programs on mental health for Students, patients, caregivers, and the community.
10. Ensure adherence to ethical standards and maintain strict patient confidentiality.
11. Perform any other responsibilities assigned by the reporting authority for effective patient care.

Qualifications & Experience:
MSW (Medical & Psychiatry)
Experience in a hospital/mental health setting preferred
