Healthcare domain familiarity (FHIR, LOINC/SNOMED/RxNorm); privacy & compliance (HIPAA/SOC 2). Experience with caching/CDN, response caching via APIM, and advanced policy authoring. Performance engineering (profiling, SQL tuning, indexing strategies) and threat modeling.
