The Senior Clinical Pharmacist is responsible for ensuring safe, effective, and rational use of medications in both inpatient and outpatient settings. The role includes medication review, policy development, patient counseling, staff training, regulatory compliance, and participation in hospital quality and safety initiatives.
Key Responsibilities:
1. Medication Chart Review
Review medication orders (OP and IP) for appropriateness, dosage, drug interactions, and allergies.
Assess patient health status to determine whether prescribed medications meet care goals.
Evaluate the effectiveness of medications and follow patient progress to monitor therapeutic outcomes.
2. Policy & Protocol Development
Develop and update institutional guidelines for drug usage.
Monitor Adverse Drug Reactions (ADR) and medication errors.
Identify, document, and report ADRs and errors; participate in root cause analysis and preventive measures.
Report ADRs to relevant regulatory bodies (e.g., PvPI) and contribute to national pharmacovigilance programs.
3. Antimicrobial Stewardship
Support rational use of antibiotics to prevent resistance.
Monitor antimicrobial usage patterns and assist in implementing hospital antibiotic policies.
4. Inventory and Formulary Management
Maintain and update the hospital drug formulary.
Ensure availability of essential drugs and prevent stock-outs or expiry issues.
5. Compliance with Regulatory Standards
Ensure pharmacy practices comply with national guidelines (e.g., NABH, NABL, CDSCO).
Maintain documentation and conduct audits as per accreditation standards.
6. Training and Capacity Building
Train nurses and other staff on safe medication practices.
Support patient education initiatives on health maintenance, including exercise, diet, and preventive care like immunization.
7. Clinical and Patient Education
Advise patients on optimal medication use.
Apply specialized knowledge on medication action, dosing, adverse effects, and drug interactions in collaboration with the healthcare team.
Control prescribing practices, monitor patient progress, and consult on drug effects.
Collaborate directly with physicians and other health professionals to achieve optimal patient outcomes.
8. Patient Safety and Quality Initiatives
Participate in hospital committees (e.g., Drug & Therapeutics Committee, Antibiotic Stewardship Committee, Quality Committee).
Contribute to quality improvement projects related to medication safety.
Support the HIC team and conduct medical and surgical audits under the guidance of the HIC chairperson.
9. Collaboration with Teams
Work closely with Infection Control, Quality, and Risk Management teams to ensure patient safety and high standards of care.
Qualification:
Pharm.D.(Mandatory)
Skills:
Proven experience in hospital pharmacy and clinical pharmacy practice.
Strong knowledge of medications, pharmacology, and patient safety.
Excellent communication, teamwork, and training abilities.
Familiarity with regulatory standards and accreditation requirements (NABH, NAB).
