Role and Responsibilities
:
Medication Management
:
Dispense medications accurately and safely according to prescribed treatments.
Review medication orders for potential drug interactions, dosage errors, or contraindications.
Ensure proper storage and handling of medications, including controlled substances.
Compound medications when necessary (e.g., preparing intravenous solutions, sterile products).
Clinical Consultations
:
Collaborate with doctors and other healthcare providers in formulating treatment plans, providing drug-related advice based on patient conditions.
Perform medication therapy management (MTM) to optimize the effectiveness and safety of patient medications.
Provide drug information and counsel patients and healthcare providers on the proper use, side effects, and interactions of prescribed medications.
Patient Monitoring

Monitor patients for adverse reactions or drug-related problems, and intervene when necessary.
Review lab results and adjust therapy based on patient-specific factors (age, weight, renal function, etc.).
Participate in ward rounds with healthcare teams to discuss patient medication regimens.
Education and Training
:
Educate patients and their families on how to use their medications, including proper administration techniques, storage, and the importance of adherence to prescribed therapies.
Provide ongoing education and training to healthcare staff regarding new medications, therapy guidelines, and drug safety.
Inventory and Supply Management
:
Oversee the inventory of medications and supplies in the pharmacy department to ensure stock levels are maintained.
Coordinate with suppliers to ensure the availability of essential medications.
Ensure compliance with regulatory requirements for the storage and handling of medications.
Research and Development
:
Participate in clinical research studies to evaluate new medications and treatment regimens.

Qualifications for Hospital Pharmacist

Educational Requirements
:
A
Doctor of Pharmacy (Pharm.D.) degree
from an accredited institution.
B pharm and M pharm
Completion of required internships or clinical rotations in a hospital pharmacy setting during Pharm.D. training.
Licensing and Certification
:
A valid
pharmacy license
in the jurisdiction where they practice (e.g., state or country-specific licensure).
Certification in

Experience
