Responsibilities

Conduct comprehensive patient assessments, including dietary history, medical history, and nutritional status evaluations. Develop and implement individualized nutrition care plans tailored to each patient's medical condition, lifestyle, and preferences. Provide expert guidance on specialized nutrition needs such as tube feeding, weight loss programs, and geriatrics nutrition. Collaborate with multidisciplinary teams to coordinate care plans, ensuring optimal patient outcomes in acute care, long-term care, or hospital environments. Educate patients and their families on nutrition principles, dietary modifications, and lifestyle changes to promote health and wellness. Monitor patient progress through regular follow-ups and adjust nutrition strategies accordingly. Utilize Electronic Medical Records (EMR) systems efficiently to document assessments, interventions, and outcomes. Stay current with the latest research in dietetics, physiology, and clinical nutrition to ensure evidence-based practice.

Qualifications

Registered Dietitian Nutritionist credential from an accredited program and licensure where required. Proven experience in hospital or healthcare settings with a focus on diabetes, lifestyle diseases and weight management. Strong knowledge of physiology related to digestion, metabolism, and disease processes impacting nutrition. Skilled in patient assessment techniques and creating comprehensive care plans for diverse populations. Experience with health coaching techniques to motivate behavioral change and support weight loss initiatives. Familiarity with EMR systems for documentation and communication within healthcare teams.
