Job Description
Qualification - BSW/MSW
Assess patients’ socio-economic conditions and provide required emotional and financial support.
Liaise with NGOs, charitable organizations, and government schemes (Ayushman Bharat, MJPJAY, etc.) for patient assistance.
Provide counseling and support to patients and families dealing with long-term or terminal illnesses.
Assist in discharge planning, post-discharge support, and patient education.
Document patient support cases, maintain social service records, and submit regular reports.
Coordinate with hospital departments for timely interventions in social cases.
Respond to medico-legal or vulnerable patient cases with confidentiality and empathy.
