Ensure proper documentation and referral planning.
Monitor the quality of assessments across designated blocks.
Therapeutic Interventions
Develop rehabilitation plans in coordination with PT/OT/ST/Psychology professionals.
Provide direct therapy sessions and home-based guidance.
Train parents and caregivers for continuity of therapy at home.
Develop modules, flyers, and training materials for stakeholders.
Medical Support
Organize medical camps in coordination with block-level teams.
Coordinate with specialists (Orthopedic, Pediatric, Neurology, Ophthalmology).
Ensure follow-up and implementation of medical recommendations.
Aids & Appliances (AAT Camps)
Coordinate the measurement, procurement, and distribution of assistive devices.
Facilitate linkages with DDRC, ALIMCO, and the Education Department.
Ensure follow-up on the effective usage of aids and appliances with children and parents.
Monitoring, Documentation & Team Collaboration
Maintain detailed records of assessments, interventions, and trainings.
Submit monthly, quarterly, and annual reports as per project requirements.
Document and share case studies, best practices, and key learnings.
Promote collaboration and coordination among team members for smooth program implementation.
