Medication Therapy Management:
Reviewing and verifying medication orders for accuracy and appropriateness.
Collaborating with healthcare providers to optimize drug therapy and resolve any medication-related issues.
Clinical Pharmacy Services:
Participating in multidisciplinary rounds to provide input on patient drug therapy.
Monitoring and assessing patient response to medications, making recommendations for adjustments as needed.
Drug Information and Education:
Providing drug information to healthcare professionals, patients, and their families.
Educating patients on proper medication use, potential side effects, and other relevant information.
Pharmacy Operations:
Overseeing pharmacy operations, including inventory management, ordering medications, and maintaining appropriate stock levels.
Ensuring compliance with regulatory requirements and maintaining a safe and efficient work environment.
Compounding and Preparation:
Compounding and preparing medications when needed, adhering to aseptic techniques.
Ensuring the proper storage and handling of medications, including controlled substances.
Quality Assurance:
Participating in quality assurance programs, conducting medication use evaluations, and implementing improvement initiatives.
Collaborating with the healthcare team to promote patient safety and quality of care.
Documentation:
Maintaining accurate and complete patient records related to medication therapy.
Documenting interventions, recommendations, and other relevant information.
Qualifications:
Degree/Diploma from an accredited pharmacy program.
State licensure as a registered pharmacist.
Hospital pharmacy experience is often preferred.
Strong knowledge of pharmacology, therapeutics, and drug interactions.
Excellent communication and interpersonal skills.
Attention to detail and a commitment to patient safety.
Ability to work collaboratively in a multidisciplinary healthcare team.
