Qualification: Degree in Pharmacy: Bachelor's or Doctor of Pharmacy (PharmD) degree from an accredited pharmacy program.
Registration with the State Pharmacy Board: Pharmacists must register with the state pharmacy board to practice in a hospital setting.
Primary Responsibilities:
1. Dispensing Medications: Fill prescriptions and dispense medications to patients, ensuring accuracy and safety.
2. Patient Counseling: Provide patients with information about their medications, including dosage instructions, potential side effects, and interactions with other medications.
3. Medication Therapy Management: Monitor patients' medication regimens and provide guidance on optimizing therapy.
Clinical Responsibilities:
1. Medication Review: Review patients' medication profiles to identify potential issues, such as drug interactions or allergies.
2. Disease Management: Provide education and guidance to patients on managing chronic diseases, such as diabetes, hypertension, and asthma.
3. Immunizations: Administer vaccinations and provide education on immunization schedules.
Administrative Responsibilities:
1. Inventory Management: Manage inventory levels of medications and supplies.
2. Prescription Processing: Process prescriptions, including verifying patient information and checking for potential errors.
3. Billing and Insurance: Handle billing and insurance claims for patients.
Other Responsibilities:
1. Quality Assurance: Participate in quality assurance activities, such as monitoring medication errors and near misses.
2. Continuing Education: Stay up-to-date with the latest developments in pharmacy practice by attending continuing education programs.
3. Collaboration with Healthcare Team: Work collaboratively with other healthcare professionals, such as physicians and nurses, to provide comprehensive patient care.
