Job Purpose

The Patient Care Coordinator (PCC) is an Assistant Manager-level role focused on the seamless execution of home healthcare services. This is a field-heavy role requiring daily travel to meet clients and staff, ensuring personalized care delivery at the point of service.

Key ResponsibilitiesField Operations & Client Engagement

Conduct daily face-to-face meetings with patients and families to assess needs and ensure service quality Perform regular on-site visits to supervise Care Assistants and Nursing Assistants Conduct shadow home visits to audit quality of care and identify improvement areas Travel approximately 80 km per day across Bengaluru to manage assigned zones
Operational Leadership

Oversee the complete home healthcare service cycle from initial assessment to ongoing delivery Monitor appointments and ensure appropriate staff deployment based on patient medical stability Handle real-time escalations from patients and staff in the field Act as deputy to the Operations Manager when required
Reporting & Documentation

Ensure completion of medical case files, consents, and progress notes as per defined TATs Prepare weekly reports using Google Sheets and Excel on patient care efficiency and first-visit conversions Coordinate with the finance team for patient invoicing and payment collections
Person SpecificationEssential Requirements

Bachelor's degree in Healthcare, Business, or a related field Experience in healthcare administration and meeting service targets Fluency in Kannada, Tamil, English, and Hindi Willingness to travel extensively (approx. 80 km/day) Proficiency in Microsoft Excel, Google Sheets, and CRM tools

Strong interpersonal and communication skills High standards of integrity and confidentiality Ability to resolve operational cancellations and staffing challenges
Work Schedule

9-hour shifts with flexible timings between 8:00 AM and 8:00 PM On-call availability required for urgent patient or staff escalations
