Altor Hospitals is seeking compassionate, organized, and proactive
Patient Care Coordinators
to act as a liaison between patients, their families, and various hospital departments. The Patient Care Coordinator ensures that every patient receives timely, coordinated, and quality care throughout their journey at the hospital.
Key Responsibilities:
Act as a single point of contact for patients and their families for all service-related support.
Assist with patient admissions, transfers, and discharge planning in coordination with clinical teams.
Coordinate between doctors, nurses, diagnostics, billing, and support services to ensure smooth patient flow.
Educate patients and families about hospital procedures, treatment plans, and follow-up care instructions.
Help patients navigate medical, administrative, and financial processes (e.g., insurance, billing, TPA coordination).
Monitor and ensure patient comfort, timely investigations, and appointments.
Collect feedback from patients and report concerns or complaints to the relevant departments for resolution.
Maintain and update patient records accurately in hospital systems.
Ensure all departments are aligned with the patient's treatment plan to avoid delays and duplication.
Promote patient rights, confidentiality, and satisfaction.
Qualifications & Skills:
Bachelor's degree in Healthcare Management, Nursing, Life Sciences, or any relevant field (Master's preferred).
1-3 years of experience in a similar role within a hospital or healthcare setup.
Strong interpersonal and communication skills in English, Hindi, and local language.
Excellent organizational and problem-solving abilities.
Knowledge of hospital management systems (HMS) and basic medical terminology.
Empathy, patience, and a service-oriented attitude.
Work Schedule:
Day Shifts, including weekends and public holidays as per hospital duty roster.
