The Patient Care Coordinator serves as a liaison between patients, healthcare providers, and administrative staff to ensure seamless, high-quality care delivery. This role is responsible for managing patient communications, scheduling appointments, coordinating care plans, assisting with insurance and billing inquiries, and supporting patients throughout their healthcare journey.
Key Responsibilities:
Serve as the primary point of contact for patients throughout their care experience
Schedule and confirm appointments, tests, and follow-ups
Coordinate referrals and prior authorizations as required
Communicate effectively with clinical staff to ensure care plans are understood and executed
Provide patient education regarding procedures, treatments, and available services
Assist patients with insurance verifications, billing questions, and financial counseling
Maintain and update patient records accurately in the electronic health record (EHR) system
Monitor patient progress and follow up to ensure timely services and satisfaction
Address patient concerns and escalate unresolved issues appropriately
Promote a patient-centered, empathetic approach to healthcare delivery
Qualifications:
High school diploma or equivalent required; Associate's or Bachelor's degree in healthcare administration or related field preferred
1-3 years of experience in a healthcare setting, preferably in care coordination or patient services
Familiarity with EHR systems (e.g., Epic, Cerner) and basic medical terminology
Strong communication, interpersonal, and problem-solving skills
Ability to multitask in a fast-paced environment and maintain attention to detail
Knowledge of insurance processes, HIPAA compliance, and patient confidentiality practices
