Care Bridge Home is seeking a skilled and compassionate
Nursing Service Manager
to lead and coordinate our Home Healthcare Services. The ideal candidate will possess strong clinical knowledge, leadership qualities, and management expertise to ensure seamless and high-quality care for all patients.
Applicants with prior experience in home care service, building community networks and healthcare collaborations will be given preference.
Only candidates with a BSc or GNM Nursing degree and relevant experience
will be considered.
Key Responsibilities
Manage and coordinate home care services.
Ensure effective communication among patients, families, caregivers, and healthcare professionals.
Provide induction, training, and continuous support to care staff to maintain high standards of clinical excellence.
Develop and implement individualized patient care plans based on thorough assessments and evolving needs.
Manage staff scheduling, work allocation, and patient follow-ups to ensure efficient service delivery.
Monitor patient progress and take prompt action to address medical or care-related issues.
Maintain accurate and up-to-date documentation of all patient interactions and healthcare activities.
Conduct hospital and community visits as needed to support service delivery.
Engage with hospitals, clinics, and other institutions to generate patient referrals and strengthen collaborations.
Build and nurture partnerships with healthcare providers to expand service reach and quality.
Promote Care Bridge Home's services through professional outreach and networking initiatives.
Identify growth opportunities and contribute to the strategic development of healthcare programs.
Maintain consistent communication and follow-up with clients, hospitals, and partners to foster lasting relationships.
Organize meetings and presentations with prospective clients and stakeholders to highlight services and potential partnerships.
Requirements
Education:
BSc/GNM in Nursing (BSc Nursing preferred)
Experience:
Minimum 1 year of experience in a healthcare or home care setting
Strong leadership, organizational, and problem-solving skills
Excellent communication in
Malayalam and English
Ability to multitask and perform effectively in dynamic clinical environments
Willingness to travel to meet the responsibilities
Must be
locally based or willing to relocate to Kochi, Ernakulam (Kerala)
