Job Summary:
The Nursing / Medical Coordinator is responsible for coordinating patient care services, managing nursing staff deployment, ensuring clinical quality, and acting as a bridge between patients, nurses, doctors, and internal teams. The role ensures smooth delivery of home health care services with high standards of patient safety and satisfaction.
Key Responsibilities:
Patient Care Coordination:
- Receive and process patient care requests.
- Assess patient requirements as per doctor's prescription.
- Coordinate initiation, continuation, and closure of home care services.
Nursing & Staff Management:
- Allocate nurses, caregivers, and technicians as per patient needs.
- Prepare duty rosters, shift planning, and replacements.
- Monitor nurse attendance, duty compliance, and conduct.
Clinical & Quality Monitoring:
- Ensure nursing care is delivered as per SOPs and doctor's orders.
- Monitor patient progress and escalate clinical concerns.
- Conduct follow-ups with patients and families.
Coordination & Communication:
- Act as a point of contact between patients, nurses, doctors, and hospitals.
- Coordinate hospital discharges and referrals.
- Liaise with HR, operations, and billing teams.
Documentation & Reporting:
- Maintain patient records, nursing notes, and consent forms.
- Prepare daily/weekly operational reports.
Complaint Handling:
- Address patient and family concerns.
- Resolve staff-related issues and escalate when required.
Eligibility & Qualifications:
- GNM / BSc Nursing (Mandatory)
- Valid Nursing Council Registration
- 1-5 years of relevant experience
Skills:
- Strong communication and coordination skills
- Clinical knowledge
- Basic computer proficiency
- Problem-solving ability
Reporting To:
Nursing Head / Operations Manager
Job Types: Full-time, Permanent
Pay: ?20,000.00 - ?35,000.00 per month
