Responsibilities:

Clinical Screening - Conduct clinical screenings to determine CCM eligibility, Utilize the EHR to document screening findings and eligibility status. Collaborate with care coordination teams to transition eligible patients into the CCM program Updating and ensuring the accuracy of patient health records by adding new information about diagnoses, procedures, medications, and other relevant data by listening the recorded patient calls Ability to work in US shifts 5PM- 2:30AM IST Stay updated on medical terminology, procedures, and treatment protocols to ensure accuracy and consistency in transcriptions Reviewing medical records to ensure they are complete and accurate. Diagnosing medical conditions based on symptoms presented in the medical record. Performing administrative tasks such as typing reports and maintaining databases.

Skills and experience :

Post graduate(Nursing) mandatory

5-10 Years over all experience( Registered nurse experience: 5 years)

2 Years clinical experience ( Bed side)

Experience of US Health care systems is a mandatory requirement. Experience in Hospital Management System or Hospital Information System experience. Commitment to maintaining confidentiality and adhering to ethical standards in handling sensitive patient information. Ensure the integrity and confidentiality of patient information by adhering to HIPAA regulations and privacy policies. Excellent English professional writing skills; including advanced proficiency in grammar and spelling. Excellent communication & listening skills Ability to understand diverse accents and dialects of physicians, their staff and patients. Familiarity with medical terminologies & medical summarization. Ability to work independently in a secure and private location with a reliable high-speed internet connection. Ability to participate in live video chat and screen sharing sessions for training. Experience with the Microsoft Office 365 or other cloud-based productivity tools.
