Maintain and update patient medical records (IP/OP) in compliance with hospital policies.
Ensure accuracy and completeness of all documents and reports.
File, retrieve, and issue medical records as per requests from clinical and administrative departments.
Enter patient data into Hospital Information System (HIS) and manage digital/physical records.
Support discharge record compilation and verify completeness.
Ensure confidentiality and secure handling of all health records.
Assist in medical coding (ICD/Procedure codes), if applicable.
Provide records for audits, legal requests, and insurance purposes following authorization protocols.
Monitor retention schedules and support safe archiving/disposal of old records.
Support compliance with NABH, JCI, or other accreditation requirements.
Requirements:
Diploma or Certification in Medical Records Technology or Health Information Management (preferred).
Familiarity with hospital documentation practices and basic medical terminology.
Strong attention to detail and organizational skills.
Ability to maintain confidentiality and professionalism at all times.
