Job Description

Job Purpose:



To manage and maintain patient medical records and ensure their accuracy, confidentiality, and timely availability in accordance with hospital policies and legal standards.

Key Responsibilities:



Medical Records Management:

Maintain and organize physical and electronic health records. Ensure completeness, accuracy, and timely filing of all patient records. Assemble and analyze patient records for completeness and accuracy after patient discharge.

Data Entry & Documentation:

Accurately enter patient data into Hospital Information System (HIS). Index records and assign appropriate codes as per ICD (International Classification of Diseases) guidelines.

Record Retrieval & Archival:

Retrieve patient files on request from authorized personnel. Archive old records systematically and ensure secure storage. Monitor and implement record retention policies.

Compliance & Confidentiality:

Ensure compliance with data protection laws and hospital policies. Maintain confidentiality and security of all medical records. Respond to requests for medical information from patients, physicians, legal entities, etc., after verifying proper authorization.

Reporting & Auditing:

Assist in preparing monthly/quarterly reports on departmental performance. Cooperate with internal and external auditors during audits.

Quality Assurance:

Conduct periodic checks for missing or incomplete documentation. Coordinate with medical and administrative staff to correct discrepancies.

Team Coordination:

Assist MRD Manager in training and supervising junior staff. Support cross-functional coordination with departments such as billing, nursing, and IT.

Key Skills & Competencies:



Attention to detail and accuracy Knowledge of medical terminology and coding standards (e.g., ICD-10, CPT) Familiarity with Hospital Information Systems (HIS) Confidentiality and integrity Time management and organizational skills Strong communication and interpersonal abilities

Educational Qualification:



Bachelor's degree in Health Information Management / Life Sciences / Medical Records Technology or equivalent Certification in Medical Records or Health Information is a plus
