, ensuring that all patient health information is accurate, stored securely, and easily retrievable. The Incharge is responsible for maintaining the hospital's clinical data integrity and ensuring 100% compliance with legal, statutory, and accreditation standards (e.g., NABH).
Key Responsibilities1. Departmental Management
Workflow Oversight:
Manage the end-to-end process of record collection, indexing, coding (ICD-10), and archiving for both Inpatient (IPD) and Outpatient (OPD) files.
Retrieval Systems:
Ensure a "zero-delay" system for retrieving records for follow-ups, research, or legal requests.
Digitization:
Lead the transition or maintenance of Electronic Medical Records (EMR) and oversee the scanning/digitization of physical files.
2. Compliance & Quality Assurance
Auditing:
Conduct regular "File Audits" to check for deficiencies (missing signatures, incomplete discharge summaries, etc.) and coordinate with doctors for completion.
Statutory Norms:
Ensure the department meets all state and national healthcare regulations regarding record retention and destruction.
Accreditation:
Maintain all documentation required for
NABH
or other quality certifications.
3. Medico-Legal & Data Privacy
MLC Management:
Act as the custodian for Medico-Legal Case (MLC) records, ensuring they are stored under high security and produced in court as required.
Confidentiality:
Enforce strict HIPAA or local data privacy protocols to ensure no unauthorized access to patient information.
4. Reporting & Statistics
Data Analysis:
Generate daily, monthly, and annual hospital statistics (e.g., Bed Occupancy Rate, Death Rate, Discharge Trends).
Management Reporting:
Present data-driven insights to hospital leadership for administrative planning and budgeting.
