of medical records as per ICD-10/ICD-11 standards.

as per statutory guidelines.
2. Data Accuracy & Documentation
Verify accuracy and completeness of patient-related documents:
Discharge summaries
Consent forms
Operative notes
Investigation reports
Ensure records are free from errors, overwriting, or missing signatures.
3. Confidentiality & Security
Implement strict confidentiality protocols for patient information.
Ensure safe and secure storage (physical or digital) of all medical records.
Restrict unauthorized access to records.
4. Retrieval & Reporting
Provide
timely retrieval
of patient records for clinical review, audits, TPA/Insurance, legal cases, or medico-legal purposes.
Prepare
daily, weekly, and monthly MRD reports
, such as:
Admission & discharge statistics
Bed occupancy
Mortality & morbidity data
Birth & death records submission
5. Compliance & Standards
Ensure compliance with
NABH, ISO, and regulatory requirements
.
Maintain complete documentation for audits.
Handle
Medico-Legal Case (MLC)
files properly and coordinate with police/government as required.
6. Coordination
Work with clinical teams, TPA, billing, and nursing to obtain missing documents.
Coordinate with IT for EMR/HIS-related issues.
Support the Quality department during audits.
7. Staff Supervision
Supervise MRD clerks/executives.
Conduct training on documentation standards and record-keeping protocols.
