Collect, compile, and file patient medical records (IP, OP, emergency, discharge summaries).
Maintain confidentiality and security of all patient records.
Ensure proper indexing, coding (ICD-10/ICD-11), and classification of diseases/procedures.
Prepare and update patient case sheets, discharge summaries, and consent forms.
Retrieve records for clinical audits, medico-legal cases, research, and management reporting.
Monitor completeness of patient files as per NABH / statutory guidelines.
Support doctors, nurses, and administrative staff with medical record requirements.
Generate periodic reports (admission, discharge, mortality, morbidity, medico-legal cases).
Assist in digitalization / Electronic Medical Record (EMR) management.
Maintain records in compliance with government regulations (e.g., MLC, Birth & Death Registration).
