Maintain patient files (inpatient & outpatient) in proper order.
Ensure completeness of records with necessary documentation, consent forms, discharge summaries, investigation reports, etc.
Check for missing or incomplete information and coordinate with nursing/clinical staff for rectification.
Coding & Indexing
Assist in coding diseases, procedures, and treatments using ICD coding systems (if applicable).
Classify and index records for easy retrieval.
Confidentiality & Security
Safeguard patient records as per hospital policy.
Ensure that records are accessed only by authorized personnel.
Data Entry & Digitization
Enter patient demographic and clinical details into the hospital information system.
Support in digitization of medical records and electronic health records (EHR) maintenance.
Legal & Compliance
Maintain records as per statutory requirements.
Provide records for medico-legal cases (MLC) with proper authorization.
Reporting & Coordination
Generate reports related to admissions, discharges, and patient statistics.
Coordinate with doctors, nurses, and administrative staff for record-related needs.
Skills & Competencies Required:
Good knowledge of hospital record-keeping systems.
Familiarity with medical terminology and ICD coding (preferred).
Strong organizational and documentation skills.
Attention to detail and confidentiality.
Computer proficiency (MS Office, Hospital Information Systems).
