Maintain, organize, and update patient medical records (both physical and digital).
Ensure completeness of patient files by verifying that all required documents are present.
Index and code medical records appropriately for easy retrieval.
Ensure confidentiality and secure storage of all records as per hospital policy and legal standards.
2. Documentation & Reporting
Prepare daily, weekly, and monthly reports related to admissions, discharges, deaths, and births.
Maintain records for medico-legal cases (MLC) and ensure proper documentation.
Assist in audits and ensure compliance with NABH/NABL/JCI requirements.
3. Coordination & Communication
Coordinate with doctors, nurses, and other departments to collect and complete pending documentation.
Support clinicians and management with retrieval of patient files whenever needed.
Respond to authorized requests for medical records from patients, insurance companies, and legal authorities.
4. Data Entry & Digital Records
Enter and update data in Hospital Information System (HIS).
Digitize old records and maintain proper indexing for electronic health records (EHR).
5. Compliance & Confidentiality
Ensure compliance with hospital policies, statutory requirements, and data protection laws (e.g., HIPAA if applicable).
Maintain high confidentiality while handling sensitive patient information.
Qualifications:
Graduate in any discipline (preferably Life Sciences / Health Administration).
Diploma/Certification in Medical Records Technology or Hospital Management preferred.
1-3 years of experience in MRD/Medical Records Management.
Skills Required:
Knowledge of medical terminology and hospital record-keeping standards.
Proficiency in MS Office and Hospital Information Systems (HIS).
Strong organizational skills and attention to detail.
Good communication and interpersonal skills.
Ability to maintain confidentiality and handle sensitive data.
