NABH standards
and hospital policies. The role plays a critical part in ensuring documentation completeness, data confidentiality, and timely record retrieval for patient care and audits.
Key Responsibilities:
Maintain medical records in an organized and systematic manner as per
NABH guidelines
.
Ensure safe custody of patient records and maintain strict confidentiality.
File and retrieve inpatient and outpatient records as requested by authorized staff.
Track movement of medical records using logs/registers (manual or electronic).
Assist in preparing files for internal and external audits, medico-legal cases, and NABH assessments.
Ensure completeness of discharge summaries, consent forms, and other documents before filing.
Support in digitization and scanning of records for electronic record maintenance.
Maintain records retention and destruction schedules as per hospital SOPs and NABH norms.
Assist with the generation of data for quality indicators and monthly reports.
Ensure MRD area is clean, secure, and access-controlled.
Report any discrepancies, missing files, or unauthorized access to the MRD in-charge.
Qualifications:
Minimum: 10th or 12th pass (Higher Secondary Certificate).
Prior experience in a hospital MRD or similar department preferred.
Familiarity with hospital documentation and basic understanding of NABH practices is a plus.
Skills Required:
Strong organizational and filing skills.
Basic computer literacy (MS Office, hospital ERP systems desirable).
Attention to detail and accuracy in handling records.
Ability to maintain confidentiality and integrity of patient data.
Good communication and coordination skills.
Working Conditions:
Based in a hospital environment.
May involve lifting or moving record files.
Will be required to work in shifts including weekends and holidays, based on hospital needs.
