Transcribe medical dictations from doctors and healthcare professionals into accurate written reports.
Prepare patient records including history, discharge summaries, operative notes, consultation notes, and diagnostic reports.
Review and edit transcribed documents for accuracy, grammar, and medical terminology.
Ensure correct use of medical abbreviations and terminology.
Maintain confidentiality of patient information as per hospital policies and HIPAA guidelines.
Proofread documents to ensure clarity, consistency, and completeness.
Update patient records in Hospital Information System (HIS) or Electronic Medical Records (EMR).
Identify and correct inconsistencies, missing information, or unclear dictations by coordinating with doctors.
Follow turnaround time (TAT) standards for report submission.
Maintain organized digital and physical medical records.
Comply with hospital policies, quality standards, and legal requirements.
Assist in maintaining accuracy for billing, coding, and insurance documentation.
Stay updated with new medical terminology, procedures, and transcription guidelines.
Report errors or discrepancies in medical documentation to supervisors.
