Collect, review, and organize patient health records, including medical history, diagnoses, treatments, and test results.
Ensure records are accurate, up-to-date, and properly stored in electronic health record (EHR) systems or physical files.
Assign medical codes for procedures, diagnoses, and treatments using classification systems like ICD-10 and CPT.
Maintain confidentiality and security of patient information as per HIPAA and other regulatory requirements.
Data Entry & Retrieval:
Enter and update patient data in electronic health systems.
Retrieve medical records for physicians, nurses, and authorized personnel when needed.
Process requests for medical records from patients, insurance companies, and legal entities following privacy laws.
Compliance & Quality Control:
Ensure medical records comply with hospital policies, legal requirements, and insurance guidelines.
Audit medical records for completeness, accuracy, and proper documentation.
Assist in preparing reports and statistical data for hospital administration and research purposes.
