Job Description

A Medical Records Specialist manages and maintains patient health information, ensuring records are accurate, secure, and accessible for patient care, billing, and compliance, primarily using Electronic Health Record (EHR) systems to organize data, apply medical codes, process release requests, and uphold strict privacy laws like HIPAA. Key duties involve data entry, auditing files for completeness, assigning codes (like ICD/CPT), releasing information legally, and supporting clinical staff. Key Responsibilities:

Data Management:

Create, update, and maintain patient records (paper & electronic) with histories, diagnoses, and treatments.

Accuracy & Coding:

Review records for completeness, assign clinical codes for billing/analysis, and resolve discrepancies.

Confidentiality & Compliance:

Strictly follow HIPAA and other regulations, safeguarding patient data.

Information Release:

Process authorized requests for records from patients, providers, and agencies.

System Use:

Utilize EHR systems for data entry, retrieval, and reporting.

Auditing & Support:
