1. Medical Records Management
Collect, organize, and maintain patients' medical records in a systematic and secure way.
Ensure proper documentation, coding, and classification of diseases (ICD coding).
2. Data Entry & Record Keeping
Input patient information into hospital information systems (HIS).
Maintain digital and physical records accurately and confidentially.
3. Coding & Classification
Use standard medical classification systems like ICD-10 and CPT for diagnosis and procedures.
Assist doctors and billing departments with accurate medical coding.
4. Compliance & Legal Use
Ensure records meet legal, ethical, and regulatory standards.
Help with medico-legal cases by providing certified medical documents when required.
5. Coordination
Work closely with doctors, nurses, and administrative staff.
Retrieve records for ongoing patient care or audits.
6. Quality Control
Audit records for accuracy, completeness, and compliance.
Flag any discrepancies or incomplete documentation for correction.
