Job Description

Responsibilities:-



Performing rapid test immediately after admission of a patient with proper PPE, if negative, detailed physical examination shall be done & documented by the duty doctor Discussion / communications with patient or relative on admission of the patient regarding chief complaints of the patient, previous treatment, medications, medical and surgical history, drug allergies etc. information obtained shall be documented in IPD case sheet All consultations, online/offline to be planned and carried out by duty doctor with timely follow up as suggested by external consultant All information after MDT round shall be shared and discussed with supervisors and shift Incharge Cross checking the medication charts for assessing compliance to medication management Assisting in each procedure with consultant such as ECT, EEG, etc. Documenting pre-procedure checklist for ECT Regular bedside patient visits to be done, maintaining check on patient's condition post high risk medication administration Conduction of 2 hourly ward rounds to ensure patient safety and ward decorum Shall check BP manually of all patients in morning at 7:30 AM & 4:30 PM to ensure that vitals being checked by Nursing Staff is accurate Adherence to Hospital's Infection Control Policy, respiratory Hygiene and cough etiquette Participation in Continuous Quality Improvement Programme Informing patient's family for any procedure/Investigation to be done with confirmed charges before procedure / investigation Ensuring presence in departmental meetings & training whenever called

DOCUMENTATION:-



Basic Documentation in case history sheet to be done in 24 hours & further completion to be done in 48 hrs of admission Non-drug orders, investigation orders, fluid orders to be documented in relevant columns in drug charts Documenting assessment of patient in progress notes inside patient's file (medical record) in every shift To get & put print out of online / teleconsultation consultation prescriptions in medical record Injury report to be filled immediately after admission and to be informed to relatives Ensuring proper consent for procedures such as ECTs are taken beforehand from patient's family or NR and is attached in patient's file Charting drug orders immediately after external/internal consultation so that there is no delay to medication administration Ensuring that draft of discharge summary including medications prescribed are documented before the discharge day of the patient Ensuring Update of Medicine and Diagnosis in Practo at the time of Discharge. Updation of the condition of patient upon admission / changes in patient's health or special information / external consultation details in official whatsapp group i.e. in "Updates - IP Ward" Detailed written handover of the shift to be documented over handover register. The same has to be signed both by "Handover Given by" & "Handover Received by"
