Job Description

The Social Worker will be responsible for providing psychosocial support, counseling, rehabilitation services, and community linkage for patients and their families. The role involves case management, coordinating care plans, conducting assessments, and facilitating programs to promote recovery and well-being.

Key Responsibilities

Conduct psychosocial assessments of patients and families. Develop individualized care plans in collaboration with the multidisciplinary team. Provide counseling, emotional support, and guidance to patients and caregivers. Facilitate group sessions, family meetings, and psychoeducation programs. Assist in rehabilitation planning and reintegration of patients into society. Coordinate referrals to external agencies and community resources. Maintain accurate case records, progress notes, and reports

Qualifications & Requirements

Bachelor or Master's degree in Social Work (MSW) . Strong interpersonal and communication skills. Ability to work in a multidisciplinary team. Empathy, patience, and strong ethical values.
