Review patient medical records, physician notes, operative reports, and discharge summaries Assign accurate

ICD-10-CM, CPT, and HCPCS

codes for diagnoses, procedures, and services Ensure coding complies with

AAPC/AHIMA guidelines

, payer rules, and government regulations Verify documentation supports the codes billed and query providers for missing or unclear information Maintain accuracy to reduce claim denials and support timely reimbursement Update coding knowledge regularly to stay current with annual code changes and updates Work closely with billing, auditing, and compliance teams to resolve coding-related issues Maintain patient confidentiality and follow
