The Hospital Pharmacist is responsible for ensuring the safe, effective, and rational use of medicines within the hospital. This includes reviewing prescriptions, preparing and dispensing medications, advising healthcare professionals on drug therapy, monitoring patient outcomes, and maintaining compliance with hospital policies and national regulations.
Key Responsibilities:
1. Medication Management
Review and verify prescriptions for accuracy, appropriateness, and potential drug interactions.
Prepare, compound, and dispense prescribed medications according to professional standards.
Ensure proper storage, handling, and security of pharmaceutical products.
Monitor medication usage and minimize medication errors.
2. Clinical Support
Provide evidence-based advice to doctors, nurses, and other healthcare staff on drug selection and dosage.
Participate in ward rounds and multidisciplinary team meetings to optimize pharmacotherapy.
Monitor patient responses to medications and adjust treatments as necessary.
Counsel patients on correct medication use and potential side effects.
3. Governance and Compliance
Ensure compliance with hospital policies, legal requirements, and regulatory standards (e.g., FDA, MHRA, or local health authority).
Maintain accurate and up-to-date patient medication records.
Participate in audits, inspections, and quality assurance programs.
4. Inventory and Supply Management
Manage stock levels of pharmaceuticals to prevent shortages or wastage.
Liaise with suppliers and procurement departments for timely replenishment.
Oversee the storage and disposal of controlled substances in line with regulations.
5. Education and Training
Provide training and mentorship to junior pharmacists, pharmacy technicians, and interns.
Stay updated on new drugs, treatment protocols, and best practices through continuing professional development.
Conduct educational sessions for healthcare staff on medication safety and updates.
Qualifications and Experience:
Education:
Bachelor's or Doctor of Pharmacy (BPharm / PharmD) degree from an accredited institution.
Licensure:
Registered Pharmacist with [relevant national pharmacy council or board].
Experience:
