The Hospital Pharmacist is responsible for dispensing medications, ensuring safe and effective use of pharmaceuticals, and providing drug-related information to healthcare professionals and patients. This role covers both
Outpatient (OP)
and
Inpatient (IP)
pharmacy operations, contributing to overall patient care and treatment outcomes.
Key Responsibilities:
1. Outpatient Pharmacy (OP):
Dispense prescribed medications to ambulatory (walk-in) patients.
Counsel patients on proper use of medications, possible side effects, and storage.
Verify prescription accuracy and legitimacy.
Maintain inventory and ensure availability of essential medications.
Collaborate with physicians to clarify prescriptions or suggest alternatives.
Manage prescription records and billing processes.
2. Inpatient Pharmacy (IP):
Prepare and dispense medications for hospitalized patients based on physician orders.
Ensure timely delivery of medications to wards and ICUs.
Monitor drug therapies, identify drug interactions, and recommend changes when needed.
Assist in compounding sterile and non-sterile preparations when necessary.
Participate in ward rounds and interact with medical teams.
Maintain records of controlled substances and comply with hospital protocols.
3. General Duties:
Ensure compliance with hospital policies, national drug regulations, and safety standards.
Participate in quality improvement and audit initiatives.
Maintain accurate documentation and inventory systems.
Provide drug information and training to healthcare staff as required.
Contribute to emergency preparedness (e.g., crash carts, emergency drugs).
Report adverse drug reactions and medication errors through proper channels.
