Job Description

1. Assess patients' physical and mental health at home.

2. Monitor vital signs and overall condition regularly.

3. Administer prescribed medications safely.

4. Provide wound care and change dressings.

5. Manage IV therapy and injections when required.

6. Assist patients with mobility and daily activities.

7. Develop and follow individualized care plans.

8. Educate patients and families about health conditions.

9. Teach proper medication use and side effects.

10. Monitor chronic illnesses such as diabetes or hypertension.

11. Use and maintain medical equipment at home.

12. Maintain accurate patient records and documentation.

13. Communicate patient progress to doctors and supervisors.

14. Identify changes in patient condition and report promptly.

15. Ensure infection control and patient safety.

16. Provide emotional and psychological support.

17. Respect patient privacy, dignity, and cultural values.

18. Assist in rehabilitation and recovery care.

19. Respond to emergencies and provide first aid.

20. Support family members and caregivers in care delivery.
