The Home Health Coder is responsible for reviewing, analyzing, and coding clinical documentation for home health services in compliance with CMS, OASIS, PDGM, and organizational guidelines. The role ensures accurate coding for optimal reimbursement, compliance, and quality reporting while supporting the clinical team with coding accuracy and timely documentation.
Assign accurate ICD-10 diagnosis codes for home health episodes based on clinical documentation.
Review OASIS assessments and physician orders for consistency, completeness, and coding accuracy.
Ensure coding compliance with CMS, PDGM, and home health regulatory requirements.
Communicate with clinicians to clarify documentation and resolve coding queries.
2. Regulatory Compliance
Follow all coding standards and guidelines set by CMS, NAHC, and industry best practices.
Ensure documentation supports diagnoses assigned and meets compliance requirements.
Maintain confidentiality and integrity of patient health information (HIPAA).
3. Quality Assurance & Audit Support
Participate in internal audits and quality reviews to ensure high coding accuracy.
Identify documentation gaps and provide feedback to clinical teams.
Assist in corrective actions based on audit findings.
4. Coordination & Communication
Collaborate with clinicians, QA specialists, and billing teams to ensure accurate episode preparation.
Provide coding support during case conferences and interdisciplinary meetings, if required.
5. Reporting & Productivity
Maintain productivity and accuracy benchmarks as per company policy.
Generate required coding reports and support revenue cycle optimization.
Required Qualifications
Minimum 2 years of experience
in Home Health coding.
BCHHC (Basic Home Health Coding Certificate)
- Mandatory.
Strong understanding of
ICD-10
,
PDGM
, and
OASIS E
guidelines.
Familiarity with CMS regulations and home health documentation standards.
Experience working with EMR/EHR systems (e.g., Homecare Homebase, WellSky, MatrixCare).
Skills & Competencies

Strong communication and documentation skills.

Preferred Qualifications

Prior experience with PDGM transition and documentation improvement programs.
