We are looking to onboard a medically qualified Health Manager to strengthen our Retention Team . The primary responsibility of the Health Manager will be to act as a dedicated point of contact for patients aged 70 years and above, ensuring their medical and service-related needs are met .
Key Responsibilities:
1. Placement Video Call (PAN India): o For all NA patients aged 70+, the Health Manager will join the placement video call.
o She will introduce herself as the Health Manager and share her official contact number with the patient's family.
o She will assure them of her availability 24/7 for any service-related concerns or medical guidance.
2. Service Escalation & Primary Medical Support:
? In case of any service issue, she will act swiftly to coordinate with the concerned team and resolve it.
? Being medically qualified, she will assess basic medical complaints and provide initial guidance on whether the patient requires a Doctor Home Visit, Teleconsultation, or Hospital visit.
3. Retention Follow-Ups:
o Post-placement follow-ups on the 3rd day, 7th day, and then weekly (as per the patient family's convenience).
o These calls will focus on overall service feedback, the patient's health status, and addressing any concerns.
o She can refer to the other available forms in the patient portal, offer basic medical advice, and update/guide the Nursing Attendant accordingly.
4. Staff Coordination:
o Weekly interaction with the assigned staff to take feedback on the patient's condition.
o She will keep the patient's family informed about the same to ensure transparency and continuity of care.
5. Cross-Selling of Portea Services: Based on patient needs, she can recommend and help arrange other relevant Portea services.
