Cleft Field Worker - Outreach & Patient Care Coordinator
Purpose of the Role:
To identify, counsel, and support cleft lip and palate patients from underserved areas; ensure timely referral to hospital for treatment; and organise community outreach activities, including cleft awareness camps, in collaboration with NGOs, healthcare providers, and local leaders.
Key Responsibilities:
Actively search for cleft lip and palate cases in assigned regions through village visits, health centre connections, and word-of-mouth networks.
Build relationships with community leaders, school teachers, ASHA/Anganwadi workers, and local clinics for referrals.
Explain the condition, treatment timeline, and available resources to patients and families in local language.
Arrange patient appointments and ensure they reach the designated hospital for screening, surgery, and follow-up.
Plan and conduct cleft awareness and screening camps in rural or underserved areas.
Organise or facilitate patient travel to and from the hospital.
Distribute posters, leaflets, and digital awareness material in villages and health centres.
Conduct small group talks in schools, panchayats, and community events.
Maintain accurate records of patients identified, referred, treated, and followed up.
Submit monthly progress reports to the program supervisor.
Work closely with NGOs, government health workers, and CSR initiatives to increase outreach.
Build long-term referral channels in high-need areas.
Ensure patients return for follow-up visits and additional surgeries as per the treatment plan.
Maintain patient privacy and uphold professional integrity at all time.
Qualifications & Skills Required:
Minimum: 12th pass; Graduate preferred.
Experience in community health, social work, or NGO field operations.
Fluency in Marathi and basic English/Hindi.
Good communication and interpersonal skills.
Ability to travel extensively in rural/remote areas.
Empathy, patience, and commitment to social service.
Performance Indicators (KPIs):
Number of patients identified and brought to treatment.
Number of outreach camps organised per quarter.
Successful follow-up rates for operated patients.
Community awareness activities conducted.

Freshers - Rs 18,000- Rs 22,000
Experienced -Rs 25,000 - Rs 32,000
