Job Description

Location : Apollo Hospitals, Swargate, Pune

Education : M. Pharma (Pharmacology) or Pharm D (Doctor of Pharmacy)

Working :

Auditing all the in-patient order for drug name, strength, formulation, dosage and duration of therapy, therapeutic duplication and to check for possible drug interaction, if any.

1. Checking the physician's drug orders for appropriateness in the patient care areas and rectification of any medication error observed with appropriate documentation as per hospital protocol.

a. Drug, dose, route, frequency

b. Drug-drug and drug-food interactions

c. Variation from organizational criteria for use

d. Weight and other physiological information

e. Therapeutic duplication

f. Potential allergies

g. Other contraindication

Monitoring of adverse drug reaction

Recording the medication errors of all type prescription errors, transcription errors, administration errors and dispensing errors as per hospital policy daily.
