Key Responsibilities:
Review and validate medication orders for accuracy, safety, and appropriateness.
Provide clinical pharmacy services including medication therapy management and drug utilization review.
Monitor patient progress, identify drug-related problems, and recommend therapy modifications.
Participate in clinical rounds with the medical team to provide pharmacotherapy insights.
Counsel patients on medication usage, dosage, side effects, and adherence.
Ensure proper documentation of interventions and maintain medication records.
Collaborate with physicians, nurses, and other healthcare staff to optimize patient outcomes.
Oversee storage, handling, and dispensing of medications as per hospital policies and regulatory standards.
Participate in antibiotic stewardship and pharmacovigilance programs.
Conduct medication audits, drug information services, and assist in formulary management.
Provide training and guidance to pharmacy interns and junior staff.
Required Qualifications & Skills:
Pharm.D (Doctor of Pharmacy) from a recognized institution.
Strong knowledge of pharmacotherapy and clinical pharmacy practices.
Excellent communication and patient counseling skills.
Ability to work effectively within a multidisciplinary healthcare team.
Attention to detail and strong analytical skills.
Familiarity with hospital information systems (HIS) is an advantage.
Experience:
Freshers or experienced candidates may apply (as per hospital requirement)
