Role Overview

We're looking for a hands-on Diabetes Program Lead to own the end-to-end, on-ground rollout of Diabetes 360 across assigned geographies. You'll build and mentor field teams, activate community partnerships, ensure protocol fidelity, and turn data into action--so patients actually get healthier, not just "registered."

What You'll Do

Own the rollout: Plan, launch, and scale Diabetes 360 sites (camps, clinics, CHW routes, partner locations) with clear weekly/monthly targets.

Community & ground engagement: Build trust with Panchayats, SHGs, ASHAs/ANMs, local NGOs, employers, and facility leads; co-design outreach calendars and referral loops.

CHW & field team enablement: Recruit, train, and coach CHWs and field executives on screening workflows (RBS/FBS/HbA1c), risk stratification, counseling, device use, and digital tools.

Patient journey & retention: Drive enrollment, follow-ups, and adherence via reminders, group sessions, and family engagement; reduce default/loss-to-follow-up.

Quality & protocols: Ensure SOP compliance for screening, confirmation, treatment initiation/referral, foot/eye checks, and emergency escalation.

Data?action: Monitor program dashboards, troubleshoot bottlenecks, and run quick PDSA cycles to improve conversion from screening?diagnosis?treatment?control.

Partnerships & linkages: Coordinate with government facilities, private providers, labs, and pharmacies for confirmatory tests, referrals, and subsidized access.

Campaigns & events: Plan monthly community drives (e.g., Diabetes Day), lifestyle sessions, and workplace screenings; ensure adequate IEC materials.

Supply & logistics: Ensure availability/calibration of glucometers, strips, lancets, BP cuffs, weighing scales, and IEC; maintain stock registers.

Documentation & reporting: Submit timely field reports, case studies, and incident notes; maintain accurate records in WHIMS/app and paper backups.

Risk & safety: Uphold consent, privacy, infection prevention, waste disposal, and incident reporting standards.

Cross-functional coordination: Work with Product/Tech for app feedback, with M&E for studies, and with Finance/Ops for vendor and claim processes.

Must-Haves

Deep "ground sense" & community chops--experience building programs in villages/urban slums and mobilizing local leaders and beneficiaries.

4-7 years of field operations/program management in public health or allied development sectors.

Team leadership experience (hiring, training, rostering, coaching) for CHWs/field staff.

Comfort with targets, field travel (60-70%), and weekend events when needed.

Strong documentation and basic data skills; able to use mobile apps/dashboards and WhatsApp for operations.

Fluency in local language(s) and working English; cultural sensitivity and empathy.

Nice to Have

Prior diabetes program management (screening, HbA1c workflows, lifestyle counseling, MDRP coordination) or broader NCD program experience.

Clinical or paramedical background (GNM/BSc Nursing/Pharmacist/MPH) or certified diabetes educator.

Experience with government convergence (NHM, NCD clinics), CSR programs, or insurance-linked health services.

Familiarity with digital health platforms, remote monitoring, and behavior-change interventions.

Success Metrics (KPIs)

Screening coverage & positivity rate by catchment.

Conversion rates: screening ? confirmation ? enrollment ? treatment initiation.

Glycemic control: % of enrolled patients achieving target control at 3/6 months.

Follow-up adherence & default rate reduction.

CHW productivity (daily valid screenings, counseling sessions, referrals closed).

Patient experience (NPS/CSAT) and community partner satisfaction.

Data completeness, SOP adherence, and incident-free operations.

Qualifications

Bachelor's in Public Health, Social Work, Life Sciences, or similar; MPH preferred.

Valid two-wheeler license and ability to travel extensively.

Right to work in India.
