Qualification:

Skills & Competencies:

Medical Knowledge:

Strong understanding of medical terminology, anatomy, and physiology.

Communication Skills:

Excellent written and verbal communication skills to interact effectively with healthcare staff and document findings clearly.

Technical Proficiency:

Proficiency in Electronic Health Record (EHR) systems, medical coding software, and MS Office.

Analytical Skills:

Ability to assess medical records for accuracy, completeness, and compliance with healthcare standards.

Job Responsibilities:

Chart Review:

Examine patient medical records to ensure completeness, accuracy, and compliance with regulatory standards. Verify correct coding of diagnoses, treatments, and procedures.

Compliance & Quality Assurance:

Ensure adherence to healthcare regulations (e.g., HIPAA, ICD-10) and institutional guidelines. Maintain high standards of documentation accuracy and integrity.

Data Validation:

Confirm that all relevant medical data, including physician notes, test results, and patient histories, are appropriately documented and valid.

Coding Review:

Cross-check procedures, diagnoses, and treatments for accurate coding, including ICD-10, CPT, and HCPCS codes.

Collaboration:

Work closely with healthcare providers, including physicians, nurses, and medical coders, to address discrepancies or incomplete documentation.

Audit Support:

Assist in internal and external audits by providing necessary documentation and clarifications when discrepancies are identified.

Report Creation:

Prepare detailed reports on chart reviews, documenting issues found and steps taken to resolve them.

Patient Privacy & Confidentiality:

Ensure strict adherence to patient confidentiality policies in compliance with HIPAA and other privacy regulations.

Documentation & Record Keeping:

Maintain accurate records of chart reviews and corrective actions for future reference and audits.
