Job Description

Training and Experience
Unless the pharmacist already has one or more year of training specifically in the hematology/oncology, within the first year they will need to receive supervised training under the guidance of the Program Director in the management of hematology/oncology patients, including hematopoietic stem cell transplant recipients. Within this year the training should include but not be limited to the following topics:
- Prevention and treatment of viral, bacterial and fungal infections.
- Febrile neutropenia.
- Nausea/vomiting and mucositis.
- Treatment of acute and chronic GVHD.
- Stem cell mobilization regimens.
- High-dose chemotherapy preparative therapy.
- Long-term follow-up medication, including vaccinations.
- Prevention and treatment of other complications associated with cellular therapy, including, but not limited to, SOS, BOOP, BOOS, hemorrhagic cystitis, and iron overload.
- Pain and palliative care.
Training shall also include:
- An overview of hematology/oncology patient care, including the cellular therapy process.
- Therapeutic drug monitoring, including, but not limited to, anti-infective agents, immunosuppressive therapy, anti-seizure medications, and anticoagulation.
- Monitoring for and recognition of drug/drug and drug/food interactions and necessary dose modifications.
- Recognition of medications that require adjustment for organ dysfunction.
- Pharmacists will be involved in the development and implementation of guidelines or SOPs related to the pharmaceutical management of transplant recipients.
Designated transplant pharmacists shall participate in ten (10) hours of educational activities related to cellular therapy annually at a minimum.
Continuing education shall include, but is not limited to, activities related to the field of HPC transplantation.
Responsibilities
Will prepare/oversee the preparation of the chemotherapy.
Maintain the regulatory requirements of the pharmacy.
Will perform daily review the medication chart, intake/output etc. on a daily basis to look for drug/drug and drug/food interactions. Will also look for the need to adjust doses.
Will involve in the preparation and modification of treatment protocols and clinical practice guidelines.
Will be responsible for educating the family or the clinical team regarding medication management.
Guide the team in auditing all the indents to anticipate, prevent, and investigate medication errors of all types.
Coordinate with doctors and nurses and answer to queries related to proper use, storage, stability and over dose of drugs
Will be responsible for quality management activity related to medication management including periodic audits.
Will be responsible for maintaining the inhouse pharmacy.
Will be responsible for setting up and maintain a process for tracking, inventory, and secured storage of investigational drugs.
Monitor the restricted antibiotic consumption and participate in infection control.
Monitor proper storage, use, disposal and documentation of left over narcotics.
Maintain crash carts, imprest stock, and disaster kits.
Summarise and review the usage of selected drugs.
Routine availability: Will be available for routine working hours for 6 days a week.

Job Type: Full-time

Pay: ₹35,000.00 - ₹45,000.00 per month
