Job Description

A Clinical Pharmacist is a specialized healthcare professional who works directly with physicians, nurses, and patients to optimize medication therapy, improve health outcomes, and ensure patient safety. They conduct medication reviews, manage chronic conditions, provide drug information, and adjust dosages in clinical settings like hospitals or clinics.

Key Responsibilities

Direct Patient Care:

Review patient medical histories, monitor drug efficacy/toxicity, and adjust medication regimens.

Collaborative Practice:

Work with healthcare teams to manage patient care,, often with prescribing privileges.

Medication Management:

Identify and resolve medication errors, drug interactions, and contraindications.

Consultation & Counseling:

Educate patients, families, and staff on appropriate drug use, side effects, and adherence.

Clinical Documentation:

Record interventions and monitor outcomes in electronic health records.

Required Qualifications & Skills

Education:

Doctor of Pharmacy (PharmD) degree.

Licensure:

Current, valid state pharmacist license.

Skills:

Strong analytical, clinical, and communication skills; proficiency with electronic health records.
